Prenatal diagnostic testing – first-trimester screening at Pränatalzentrum in Stuttgart


First-trimester screening is an important complementary examination in pregnancy (an integral part of pregnancy care). It is a non-invasive procedure which does not involve any risks at all. As part of our first-trimester screening at Pränatalzentrum in Stuttgart, we perform a comprehensive ultrasound examination as well as maternal blood tests. Prenatal diagnostic testing, aided by first-trimester screening, aims to detect major malformations of the embryo or an increased (possible) risk of chromosomal disorders, for example the presence of trisomy 13, 18 or 21, in the unborn child at an early point in pregnancy.


First-trimester screening: a calculation of risks

The risk of certain chromosomal disorders depends on various factors. One such factor is the mother’s age. Other factors that may indicate a possible risk are the laboratory levels of free beta-HCG and PAPP-A, two hormones produced by the placenta. During the ultrasound examination, we also measure the embryo’s nuchal thickness and evaluate further parameters such as the nasal bone, the venous duct and the tricuspid valve. When all the results of the first-trimester screening are available at Pränatalzentrum in Stuttgart, we can ascertain the individual likelihood for the examined pregnant woman of there being a risk of chromosomal disorders in the unborn child. Based on the results of the first-trimester screening, further non-invasive or invasive examinations may be useful for further evaluation. (In case of a low risk, no further examination must be performed. Invasive procedures to be performed in case of a higher risk may include amniocentesis or chorionic villus sampling.)


The right timing for prenatal diagnostic tests


  • First-trimester screening can only be performed between the 12th (=11+) and 14th (=13+) week of pregnancy. This is because the accumulation of fluid in the nuchal area, which is evaluated at the nuchal fold test, is transient. The first-trimester screening performed as part of prenatal diagnostic testing also allows us to detect further malformations at an early point in time. Nevertheless, a detailed fetal ultrasound scan should be performed between the 20th and 22nd week of pregnancy.


Detecting possible malformations by first-trimester screening in Stuttgart

Usually, it is between the 20th and 22nd week of pregnancy that a more in-depth look is taken at the fetal organs and potential fetal malformations. However, part of this may already be done as part of the prenatal diagnostic testing in the first-trimester screening. Being experienced specialists with the required technological equipment, we are in a position to evaluate various ultrasound parameters such as nuchal translucency, the nasal bone and the tricuspid valve as well as the venous duct; please see above. In the course of the first-trimester screening, we can also make a statement regarding a potential risk of the pregnant woman developing preeclampsia (toxemia of pregnancy) or placental hypofunction. The earlier a possible risk is detected, the easier it is to initiate prophylactic or therapeutic measures (counter-measures).
